DACK G. - 1891 - 07/07/1891 – Age 38 – Sinker. Rotherham Main, J Brown and Co., Rotherham, Yorkshire. Stay broke, to which was suspended one of the chains of the scaffold, causing it to tilt when the men fell off. 4 killed.
DACK Walter - 1892 - 11/02/1892 – Age 19 – Labourer. Boldon, Harton Coal Co. Ltd., Boldon, Co. Durham. Inquiries made. Died from inflammation of the bowels caused, it was said, by a strain when lifting the bottom board of a waggon.
DADDOW Samuel John (injured) - 1890 - 26/02/1890 – Age 35 – Miner. South Wheal Crofty (metalliferous), South Wheal Crofty Mining Co., Carn Brea, Cornwall. While descending 10, a gig from the surface, a piece of wood entered the gig and pierced Daddow's side. The wood was a splinter from the side of the runner and consisted of the heart of the pitch pin of which the runner was made. The gig had been run two or three times from surface to the bottom of the mine without anything unusual occurring but on the trip, during which the accident took place, the side of the gig must have thought the splinter which curiously enough, entered between the angle iron of the frame and the plate forming the side of the gig and the weight of the latter ripped the splinter off for about 3 feet 6 inches and this finding its way through the joint entered the maxilla of the man who was sitting on a keg, on the floor of the cage, and pierced his body to the extent of 18 ins. The road had been examined shortly before and considered to be in good repair. That the accident did not prove immediately fatal was a matter of the greatest surprise.

DAGLISH John - 1894 - 05/11/1894 – Age 20 – Stoneman. St. Helens, St. Helens Colliery & Brickworks Co. Ltd., St. Helens, Cumberland. Inquiries made. The evidence given at the inquest showed that the deceased strained himself when lifting a stone and that his death was due to this cause.
DAGNALL William - 1895 - 01/06/1895 – Age 12 – Drawer. Whitemoss, White Moss Coal Co. Ltd., Rainford, Lancashire. Inspection and inquiry made. He was struck by the motion rope jerking near the far end sheave caused by a lashing chain being drawn into the pulley before the man appointed could loosen it from the lashing rope. He succumbed to the injuries received, the following day.

DALE John - 1899 - 24/02/1899 – Age 49 – Labourer. Bunkers Hill, William Rigby and Co., Staffordshire. Run over by trucks shoved by a loco. engine while crossing a line on colliery sidings.
DALE Peter - 1893 - 10/01/1893 – Age 24 – Miner. Wheal Owles (metalliferous), Thomas Bolitho Sons and Others, St. Just, Cornwall. Irruption of water, from Old Wheal Drea workings, which belonged to the same mine. The plan being inaccurate misled the agent as regards the relative position of the workings. 20 killed.

DALLARD William - 1891 - 11/04/1891 – Age 31 – Sinker. Ashton Vale Fraynes, Ashton Vale Iron Co. Ltd., Bristol, Gloucester. He was with others engaged enlarging a shaft and when ascending, in a "hoppet", from a shot he caught under a ‘byatt’ and was crushed. The arrangements were not of a satisfactory character.

DALTON Michael - 1899 – 07/01/1899 – Age 60 – Dayman. Pemberton, J Blundell and Sons, Wigan, Lancashire. Inspection made. He was working at the foot of a cage elevator, on the surface, and for some reason got under the fence rail when the descending cage came down upon him. He succumbed to the injuries, received, at the infirmary on the 13th.

DALY James - 1897 - 02/12/1897 – Age 28 – Dataller. Pemberton, J Blundell and Sons, Wigan, Lancashire. Inspection made and inquest attended. Whilst engaged building a pack wall, on side of a roadway near the face, they appear to have disturbed the roof by taking a running bar out. The roof afterwards gave way over the pack falling on to the roadway and crushing the deceased. The roof in that part was full of slips.
DALY John - 1899 - 26/05/1899 – Age 27 – Sinker. Newcraighall, Niddrie and Benhar Coal Co. Ltd., Edinburgh. Deceased was standing under a scaffold, in a sinking incline, when the roof suddenly fell. Two shots, fired immediately before had been bored into the roof and had broken it. The seam inclined 75 degrees.

DALZIEL George - 1899 - 04/12/1899 – Age 47 – Engineman. Grasshill No. 1, William Baird and Co., Ayrshire. When cleaning and oiling beam of pumping engine, while at rest, he slipped and fell backwards, a distance of nine feet.

DANDO G.H. - 1899 - 11/12/1899 – Age 30 – Shunter. Llwynypia, Glamorgan Coal Co. Ltd., Glamorgan. While poling a wagon, clear of a crossing on a curve, he let the pole drop before it got fast to the engine and was crushed between the wagon and engine. Died 13th December.
DANDO Henry - 1898 - 28/07/1898 – Age 25 - Timber cutter. Foxcote, Writhlington Collieries Co. Ltd., Somerset. Fatally crushed by a tree or log of timber which rolled down from a pile in consequence of its having been liberated by the removal of another below it.
DANDO James - 1896 - 12/06/1896 – Age 54 – Collier. Blaina Lower Deep, John Lancaster and Co. Ltd., Blaina, Monmouth. When removing a sprag, to get down coal, a quantity of rubbish fell on him from the roof causing fatal injuries.

Fall of roof in a stall road, where two experienced men were engaged repairing, having been sent there especially to do so. A pure error of judgement on the men’s part.

Deceased had entered the quarry and was taking away chalk by getting it from under an overhanging part of the quarry when the face slipped over and killed him. Deceased was a trespasser at the time of the accident.

DARBYSHIRE William - 1899 - 24/03/1899 – Age 46 – Collier. Moss No. 6, Pearson & Knowles Coal and Iron Co. Ltd., Wigan, Lancashire. Inspection made and inquest attended. The deceased had capped two sprags from against the coal and when attempting to get the coal down a stone fell upon him from slips in the roof running parallel with the coal face. The roof to within four feet of the face.
DARE Walter - 1890 - 21/03/1890 – Age 35 – Hitcher. Llwynpia, Glamorgan Coal Co. Ltd.,, Tonypandy, Glamorgan. Crushed by cage descending upon him while clearing rubbish from the sump during the night shift. The engineman maintained that he had received a signal to lower the cage, others at top and assistant hitcher said no signal had been heard or given.
DARLING Robert - 1898 - 22/12/1898 – Age 21 – Drawer. Newbattle, Lothian Coal Co. Ltd., Edinburgh. Deceased was taking a loaded tub down a side road dipping 1 in 5. He appeared to have been in front of the tub, in contravention of Additional Special Rule 5. The tub overpowered him and drove him against the side of the level causing injuries which he succumbed to three weeks afterwards.

Died 7th Mar. The coal fell from a slip running diagonally into the face which might have been observed but the two collieries working with deceased said they did not know it was there. 2 killed.

DART John - 1895 - 16/02/1895 – Age 24 – Banksman. Deaf Hill, Trimdon Coal Co., Co. Durham. Descending the pit, on top of cage, when two collided at meetings and he was thrown down the shaft.
DARTCH Anthony (non-fatal) - 1893 - 12/05/1893 – Age 65 – Miner. Devon Great Consols (metalliferous), Devon Great Consols Ltd., Tavistock, Devon. He was crushed while descending by the man-engine, having missed the step upon which he should have descended.
DARWOOD James - 1897 - 04/11/1897 – Age 20 – Trailer. Walkmill, Moresby Coal Co. Ltd., Cumberland. Inspection made and inquest attended. The deceased and his marrow were working at the face of a wall, 5 yards wide, along the low side of which was a 3.5 feet dip hitch and at right angles to this a slip ran across the face. About 3 feet of top coal and cash fell away at this slip breaking on at the first two props about 6 feet from the slip and crushed the deceased. The place had a short time previously been examined by the undermanager and the deputy.
DAUBER Alfred - 1892 - 12/09/1892 – Age 18 – Drawer. Ince Hall, Pearson and Knowles Coal and Iron Co. Ltd., Wigan, Lancashire. Inspection made and inquest attended. Deceased was assisting to rip an airway, that was low for the tubs, when he liberated a stone in the roof which struck him before he could get clear himself.
DAVENPORT George (injured) - 1890 - 15/10/1890 – Age 27 – Miner. Hoddlesden Half Yard, Joseph Place and Sons, Hoddlesden, Lancashire. Very slightly injured by firedamp, which lighted at his candle where he had passed the danger signal, there being a fall of roof in the airway.
DAVENPORT Joseph - 1892 - 08/02/1892 – Age 27 – Driver. Wyken, Wyken Colliery Co., Coventry, Warwick. Riding on a full tub, down an inclined roadway, when the tub got off the road and crushed the deceased against a prop. Pony working in sling gears and had no power to stop the tub.
DAVENPORT Richard - 1895 - 14/05/1895 – Age 20 – Waggoner. Outwood, Thomas Fletcher and Sons, Lancashire. Fall of coal from face came over two sprags one of which was too short and easily overridden.
DAVENPORT Samuel - 1892 - 21/10/1892 – Age 54 – Stallman. Annesley, Annesley Colliery Co., Nottinghamshire. Fall of roof. Deceased had left his working place and gone into another working place, contrary to the rules of the mine, and whilst talking to another man a quantity of the roof coal fell upon him.

Deceased and his son were taking a 6 feet cut off the side of a Stoop to form a new road the old drift having closed. He was Sitting holing at the face when a large stone fell from the roof, killing him almost instantly.

DAVIDSON James - 1897 - 25/06/1897 – Age 36 – Labourer. Dunion Whinstone (quarry), Roxburgh. A block of whinstone fell from top of quarry face, ricocheted off some loose rock and crushed deceased’s leg.

As Davidson was running alongside the tube, to get to the limbers, he came in contact with a prop, set in the middle of the flat, and fell between the tubs. It was his first day at driving at this fiat, and after he had taken the dregs out of the tubs the pony moved away.

DAVIES Benjamin - 1891 - 19/12/1891 – Age 24 - Waggon shifter. Cyfartha Cethin No. 2, Crawshay Bros. Ltd., Merthyr Tydfil, Glamorgan. He fell under a waggon and was run over while engaged with another man in shifting it from the screen during the night shift.
DAVIES Benjamin - 1897 - 29/01/1897 – Age 30 – Miner. South Halkyn Lead, South Halkyn & Rhydywym Mining Co. Ltd., Flint. While he was at the bottom of a vertical shaft, 100 yards deep, which was in the course of being sunk a stone fell upon him and inflicted injuries from which he died three days afterwards. The stone probably fell from a bucket which had just been sent up full.

DAVIES D.E. (injured) - 1890 - 06/11/1890 – Age 33 – Rockman. Park (slate). He was drying, over the smith’s fire, a tube used for charging boreholes with gunpowder, when a slight explosion occurred and drove the tube against his thigh. A little powder had remained in the tub probably from its having been damp when last used. 3 weeks off.

DAVIES David - 1893 - 20/10/1893 – Age 28 – Repairer. Cambrian Navigation, Thomas Riches and Co., Clydach Vale, Glamorgan. While repairing, a small stone fell on his head, cutting it. He died from inflammation of the brain nine days afterwards and had worked on four of these days.

Having worked the bottom coal they knocked the props out, under the bind coal, and when in the act of getting it down it gave way between two slips, part of the roof also falling down upon the deceased.

Fall of side at face coal 4 ft. x 4 ft. x 5 ft. thick, he had driven a wedge on top of it and was turning around to get a mandril when it fell and knocked him against the corner of the tram. Died five days after. Four feet seam.
DAVIES David John - 1891 - 12/09/1891 – Age 13 - Collier’s boy. Penrhiwfer, Glamorgan Coal Co. Ltd., Penygraig, Glamorgan. Deceased was going in bye to work carrying a mandril and in stepping over haulage rope, at parting, the rope was put in motion and tripped him, causing him to fall on the mandril the point of which entered his neck. He walked home and almost recovered from the injury but inflammation of the brain supervened and he died on 24th October.

DAVIES David R. - 1890 - 11/09/1890 – Age 12 - Colliers’ boy. Derllwyn, JL Smith and Co., New Tredegar, Monmouth. On coming out of the level, he was riding down the incline on a full set. In some manner, not explained, he fell in front of the trams and was killed. He should not have been riding as it is against orders and magisterial proceedings have since been instituted for a similar offence.

DAVIES Edward - 1891 - 03/06/1891 – Age 29 – Collier. Edge Green, Richard Evans and Co. Ltd., Golborne, Lancashire. Inspection made. The deceased was holing his coal on the top when he bared some slips and the roof fell upon him. The circumstance was quite unforeseen and difficult to guard against. 2.0 pm.
DAVIES Edward - 1891 - 11/02/1891 – Age 50 – Collier. Brynmally, Thomas Clayton, Wrexham, Denbigh. Inspection made and inquest attended. He was blowing up the bottom in a roadway. After examination made by the fireman he emptied several cartridges into the hole and was either pushing the powder to the back of the hole, with his copper scraper, or he had commenced to ram the shot with an iron stemmer, tipped with 4 ins. of copper, in the latter supposition the friction of the iron stemmer on the bottom of the hole might have ignited some loose grains of powder. It was decided to use only wooden stemmers in this mine and that no powder, except in cartridges, was to be used.
DAVIES Edward - 1895 - 10/03/1895 – Age 22 – Miner. Wheal Agar (metalliferous), Wheal Agar Adventurers, Cornwall. While refixing some planks, which had been disturbed during blasting operations, he appears to have slipped or overbalanced and fallen about 6 fms. on to the piles of stuff in his stope below, receiving injuries from which he died on the 21st.
