OLIVER W. - 1873 - 27/11/1873 – Signalman. Cannock and Rugeley, Cannock and Rugeley Colliery Co., Rugeley, Stafford. Overwinding of a tail rope underground snatched a horizontal pulley out of its place and stuck him killing him on the spot.

ORRELL James - 1876 - 02/08/1876 – Age 25 - Miner. Van (metalliferous), Van Mining Co., Montgomeryshire. He thought a fuse he had lighted had missed fire and returned to the hole in about two minutes and the charge exploded hurling some large stones on him resulting in injuries from which he died in about half an hour. He was cautioned by his partners that it was too soon to return but would not listen.
