OAKLEY J - 1868 - 30/10/1868 – Age 34 - Pikeman. Himley, Earl of Dudley, Kingswinsford, Staffordshire. The Chartermaster and the Deputy were trying to throw some overhanging Coal with levers when suddenly and expectedly a mass of brazils fell killing him. 2 injured.

OAKLEY John - 1860 - 12/09/1860 – Age 27 - Pikeman. Priestfield, Ward and Son, Wolverhampton, Staffordshire. Fall in fireclay coal whilst at work holing. Caused by double slips or ‘backs’.

OAKLEY John - 1860 - 16/03/1860 – Age 31 - Loader. New Mine, Parker and Halford, Wednesbury, Staffordshire. A mass of thick coal fell on him in a place badly squeezed and so full of chokedamp that it was at great peril I approached near the place.

ORACHER D. - 1867 - 28/07/1867 – Age 21 - Sinker. Tennochfide, Monkland Iron Co., Holytown, Lanarkshire. Killed by a shot in sinking pit. The match kindled the straw before they had time to get out of harms way. 2 killed.

ORCHARD James - 1865 - 06/11/1865 – Age 30 - Miner. Varteg (ironstone), Partridge and Jones, Varteg, Monmouth. Injured on 4th by fall of stuff from face of workings which pressed so severely on the region of his heart that he died on the 6th.

OSBALDASTON George - 1865 – 14/07/1865 – Age 23 - Miner. Wardley, Bridgewater Trust, Worsley, Lancashire. Suffocated by firedamp. The gas accumulated in consequence of his taking down the bratticing which sent air in. He was about to replace it, but instead of keeping air up with them he left his safety lamp behind and went right into the gas.

OSMUND Henry - 1866 - 16/01/1866 – Age 17 - Miner. Sirhowy (ironstone), Ebbw Vale Co., Ltd., Ebbw Vale, Monmouth. In No7 pit a fall of ground lying between 2 known slips at right angles the apex pointing upwards. Another slip, nor previously seen, caused the fall that killed him.

OUGHTON or HOUGHTON, George - 1866 - 31/10/1866 – Age 18 - Putter. Pelton, Lord Dunsany and Partners Chester-le-Street, Durham. Killed by an explosion of firedamp in the Busty Bank Seam. 24 killed. Of Pit Houses.

OUTRAM W. - 1868 - 07/05/1868 – Age 37 - Banksman. Waleswood, Skinner and Holford, Sheffield, Yorkshire. Repairing signal wire in the shaft bottom when the cage was lowered upon him.

OWEN D. - 1864 - 16/06/1864 – Age 36 - Sinker. Dimmacks, P Williams and Sons, Tipton, Staffordshire. The horse in the gin took fright and snapped that portion to which he was attached whilst the deceased were descending when they were immediately precipitated to the bottom of the shaft and killed. 2 killed.

OWEN Joshua - 1860 - 12/05/1860 – Age 38 – Doggy. Gospel Oak, Grazebrook and Ashton, Tipton, Staffordshire. Explosion of firedamp. Error in the level of the gateroad had caused the water to roof and when by a gutter the water came away the gas came with it and fired at a candle near bottom of the shaft. In performing this operation they should have had a lamp. 2 killed.

OWEN Morgan - 1862 - 10/10/1862 – Age 30 - Collier. Yard, Rhymney Iron Co., Rhymney, Monmouth. Fall of roof though the poor man himself considered it safe. Died 27th.
