He gave the usual signal for a empty set of tubs to be taken up a self-acting incline but forgot to attach the rope to the tubs. The man at the end took the set off the full tubs and was unable to control them, they rushed to the bottom and collided with and displaced some chocks, which allowed some girders and roof to fall and he was caught and killed by the fall.
BRUCE John - 1908 - 03/07/1908 – Age 51 - Barbauchlaw Oversman. R Muir and Co. Ltd., Linlithgow. Deceased was proceeding down a mine from the surface, behind an empty rake, when a runaway tub from the surface overtook him.

The locomotives were to be placed on different lines of rails to be ready for the drivers. Deceased and another lad rode on the last engine. After going past the points, leading to the different lines, the foreman reversed his engine and shoved the other two back; deceased, who had jumped off and shifted the points, then proceeded to uncouple the engine he had ridden on from the middle one and was crushed between the buffers. There were coupling poles on the engines, one of which he should have used.
BRUCE William - 1902 - 02/09/1902 – Age 15 - Hill of Beath Pony driver. Fife Coal Co. Ltd., Fife. Deceased was riding inbye in one of a set of four empty tubs. When passing round a sharp curve, the horse shied, and pulled the leading tub off the rails. It caught and pulled out some props, letting down the roof. He was killed instantaneously.
BRUCE William - 1902 - 15/08/1902 – Age 21 - Skellyton No 3 Miner. United Collieries Ltd., Lanarkshire. Gas, which was known to be given off, accumulated at the brushing face and was ignited by a naked light. Apparently one of the deceased had taken down a screen, and this prevented the air current from playing upon the feeder. From Main body of the report: The first fatal explosion happened in Skellyton Colliery and by it two men lost their lives. Fire-damp was known to be given off from a feeder at their road-head, and by blocking the airway, along the coal face by a screen and erecting a hurdle screen, etc., at the road head, the air current was made to play upon the brushing face and keep the gas from accumulating. The two men were alone when the explosion happened, but it is evident that they had taken down the screen placed across the face. This diverted the air current, and the fire-damp which in consequence accumulated was ignited by their naked lights. The system of trying to use naked lights in workings where fire-damp is freely given off, and at the same time prevent explosions by an elaborate use of screens, is one which is certain sooner or later to fail. Safety lamps, along with other precautions, in such circumstances, ought always to be used, and in this case naked lights ought not to have been allowed. Since the explosion safety lamps have been introduced. It appears that the fireman was not in the habit of recording in his report book the presence of fire-damp unless he found a considerable accumulation. This practice of not recording gas when found in small quantity or at feeders where there is no accumulation is too common, and it cannot be too clearly understood that a fireman is bound to record the finding of gas, however small in quantity, wherever he finds it when making his inspection previous to the commencement of the shift.
BRUCE William - 1903 - 03/04/1903 – Age 40 - Herrington Shifter. Lambton Collieries Ltd., Co.Durham. When cutting level for air pipes under mined the timber and a fall of side stone killed him.

The deceased was in the act of getting into the cage when it was lifted and he was crushed between the cage and buntons. The accident apparently arose through a mistake in the signals on the part of the engineman and banksman.
BRUNDRETT Joseph - 1907 - 21/01/1907 – Age 25 - Norton Collier. Robert Heath and Sons Ltd., Staffordshire. Deceased was pulling coal off a longwall face in a seam, 7 feet 6 inches thick, when about 2 cwt. of coal came away, struck the lever with which he was working and drove the handle of it against the pit of his stomach, his injuries were not at first considered to be serious, but he died two days afterwards apparently from internal inflammation.
BRUNS Joseph - 1900 - 26/03/1900 – Age 48 - North Seaton Sandstone Quarry, Quarry foreman. J and G Douglass, Northumberland. Inspection made and inquest attended. He was attempting, during a snow storm, to pass between a crane and the edge of the Quarry when he fell over.

Deceased and another were driving upset in a seam 8 feet 9 inches in thickness and having an inclination of 24 deg. Two shots each containing 1.5 lbs. of gunpowder and about 4 feet apart were being lighted. The gutta-percha covered fuse of deceased's shot did appear to ignite readily and as that the other shot was burning freely the men retired. On hearing one shot explode the deceased at once returned to light the other, under the impression that he had formerly failed to do so. He appeared to have been close to it when it exploded causing injuries to which he succumbed two hours afterwards.
BRUNTSILL John - 1901 - 27/11/1901 – Age 69 - Nelson Collier. William Ramsden and Sons Ltd., Lancashire. Fall of side from face of working place whilst building a pack. Face had been standing four days overhanging. Ought to have been spragged.

BRYANT Albert - 1909 - 15/11/1909 – Age 49 - Easton Ripper. Bedminster Easton Kingswood and Parkfield Collieries Ltd., Gloucester. He was employed in repairing an airway in which a fall had occurred, and apparently had crept over the fall after he had done some work at the place, and in doing so probably disturbed some props, with the result that a further fall occurred and smothered him. The roof in the "Red Ash" seam in this place is exceptionally bad and it took about eight hours to get him out.
BRYANT John - 1905 - 14/07/1905 – Age 58 - Lower Duffryn Labourer. Powell Duffryn Steam Coal Co. Ltd., Glamorgan. At face cliff, 27 feet by 5 feet wide at one end and 18 inches the other by 18 inches thick, fell as deceased was stowing rubbish. The stall was well propped but a heavy squeeze occurred and broke or discharged the timber. Seven Feet seam.
BRYANT Oliver - 1907 - 18/06/1907 – Age 20 - Hanham Hitcher up. Bedminster Eston, Kingswood and Parkfield Collieries Ltd., Gloucester. Crushed between an empty tub and a collar under the roof in this top East Siding down the South incline. This road was nearly finished and was in none too good repair. The siding is rather cramped and they had got too many tubs there for convenience. This full journey was longer than usual and empties blocked the journey going out. Deceased and others were trying to move some of the empties off the rails to clear the journey, he was between tubs and remained there after being told that the journey was going to be slackened back but unfortunately owing to a misunderstanding in signalling the journey was drawn up and he was leaning over at the end of the tub and was crushed against the collar.
BRYANY George - 1908 - 02/12/1908 – Age 59 - Kingswood Labourer. Bedminster Easton. Kingswood and Parkfield Collieries Ltd., Gloucester. While assisting a timberman in enlarging a horse road, and in a stooping position clearing the rails to allow an approaching journey of trams to pass, the arm for a pair of double timber sets temporarily fell over and struck him on the temple and knocked him down. It was not thought serious at the moment, but he became unconscious in less than three hours, and died from cerebral haemorrhage 12 hours later. A post-mortem showed that there was a fracture of the skull four inches in length, although no scratch, abrasion or discoloration was noticeable externally.
BRYCE Alexander - 1908 - 16/10/1908 – Age 30 - Common No.16 Drawer. William Baird and Co. Ltd., Ayrshire. When letting away a race of hutches, from the top of a self-acting incline, the rope was drawn out of the hose and struck him on the head.
BRYDEN William - 1909 - 13/12/1909 – Age 61 - Brora Manager. John Melville Sutherland. Died on January 10th from injuries received by an explosion of gas when making the statutory examination, under General Rule 4 of the Coal Mines Act 1887, with a naked light, on the date named. His vest, which had a calico back, was greasy from contact with the oily parts of a pump, it caught fire and he was burned about the small of his back. Gas had been seen in the same place about a fortnight before and in using a naked light the deceased contravened General Rule 4 (1). The explosion occurred in a heading which was ventilated by means of a brattice this brattice was not properly hung and there were, in consequence, places where it did not touch the roof and thus allowed the ventilating current to escape before reaching the face. It is probable that if the ventilation had been carried up to the face the gas would not have accumulated.
BRYNAT George - 1904 - 04/08/1904 – Age 27 - South Wales, Cwmtillery Brakesman. Lancasters Steam Coal Collieries Ltd., Monmouth. A spur wheel, which had shortly before been unloaded from a tram, was scotched up against a wall and suddenly fell over on him. It had been set up too perpendicularly and probably the traces of a horse, which just about then passed with a tram caught in it and caused it to fall.
BRYNE James - 1908 - 18/08/1908 – Age 36 - Maypole Contractor. Moss Hall Co., Wigan, Lancashire. Explosion of firedamp and coal dust. 75 killed.
BRYSON John - 1908 - 22/01/1908 – Age 51 - Hyndshaw Fireman. Chapel Coal Co. Ltd., Lanarkshire. Deceased was making some repairs on the dook haulage road. When rake was ascending he stood to the side to allow it to pass but the space was too narrow and he was crushed by the tubs. He had ample time to go to a manhole, 6 feet below, from the time the rake began to move up.
BUCHAN John - 1900 - 13/02/1900 – Age 34 - Hamilton Place No.1 Miner. Bent Colliery Co. Ltd., Lanarkshire. Fall of roof in working place.
BUCHANAN James - 1905 - 08/11/1905 – Age 39 - Closeburn Sandstone Quarry, Quarryman. Scottish Freestone Quarries Ltd., Dumfries. He was dressing a stone on the bank and failed to stand clear while a large stone was being shifted by a crane. The jib rope broke and the jib fell on him.
BUCHANAN John - 1902 - 04/11/1902 – Age 43 - Drum (Fireclay & Ganister) Miner. Bonnybridge Silica and Fireclay Co., Stirling. The seam is worked in three divisions, top, mid and bottom. Deceased had bored and charged a hole in the mid division, the quantity of gunpowder being about 2 lbs. He used a paper squib, made up by himself in the mine, and to lite it he attached a wick in the usual way. When everything was ready he put his light to the match, the shot then exploded just as he turned to go. He died from his injuries six days afterwards. Deceased had apparently ignited the squib instead of the match.
BUCHANNAN Thomas - 1904 - 02/02/1904 – Age 24 - Greenhill Miner. Greenhill Colliery Co. Ltd., Lanarkshire. While lifting a piece of coal at his working place it broke and a part injured the forefinger of his right hand severely. He and his brother ceased work and proceeded to the shaft bottom for home and on arriving there he complained of being faint, after getting a drink he revived and he, accompanied by his brother and another workman, stepped on to the cage to be raised and when 12 feet from the surface he again took ill and fell on the cage and before either of the men could grip him he had slipped past the side and fell to the bottom, a distance of 56 fathoms.
BUCK Thomas - 1907 - 15/08/1907 – Age 20 - Annesley Dataller. Annesley Colliery Co., Nottinghamshire. Fall of roof on road; while passing along a roadway, and when under a short length of roof, called a "bridge," which had not been ripped up to a higher bed, the lower stratum fell and killed him. Such a piece of roof should not have been left untimbered. Nine Feet Seam. Safety lamps.
BUCKLE William - 1908 - 20/05/1908 – Age 62 - Aston Dataller. Aston Coal Co. Ltd., Yorkshire. He was pulling props and then taking all loose stone down to make a road safe, and when doing so a stone 9 feet long by 3 feet wide by 17 inches thick, fell and killed him.
BUCKLEY George - 1904 - 22/12/1904 – Age 36 - Silver Hill Stoker. Stanton Iron Works Co., Staffordshire. "Died from valvular disease of the heart and dropsy" was the verdict of the coroner's jury. He received a slight injury at the mine on 26th August.
BUCKLEY J. - 1903 - 03/07/1903 – Age 18 - South Duffryn No.1, Labourer. Hills Plymouth Co. Ltd., Glamorgan. Fall of roof. Deceased was resting after discharging rubbish when a small piece of cliff fell from between two collars, three feet apart, on to his back. He died on December 11th from blood poisoning set up by a fragment of fractured bone the result of the accident.
BUCKLEY Joseph - 1903 - 16/12/1903 – Age 41 - Ashtons Green Waggon lowerer. Bromilow Foster and Co., St Helens, Lancashire. Inspection made. He appears to have asked his mate to let two waggons down, so as to bump two other ones out from under the screens. After this had been done he stepped between, to push the front ones, when the others followed on and crushed him between the buffers. Deceased had put a scotch on the line to stop the latter ones but one came over it.
BUCKLEY Oliver - 1908 - 14/12/1908 – Age 14 - South Moor, Charley Driver. South Moor Colliery Co. Ltd., Co.Durham. After holing he opened a door for his pony and set to pass through, he ran along to get on to the limbers. He, unfortunately, tried to get past the tubs on the narrow side and was caught between them and the side of the road and killed.
BUCKLEY Robert - 1901 - 17/10/1901 – Age 34 - Naden Dene Dataller. J Lomax and Co., Lancashire. Fall of roof in roadway of tunnel. Timber not strong enough and too far apart. (Bad roof). 2 killed.
BUCKNELL Richard - 1905 - 07/11/1905 – Age 42 - Denaby Main Dataller. Denaby and Cadeby Main Collieries Co. Ltd., Yorkshire. Deceased was engaged repairing an airway and filled the stone into coal tubs. The gradient where he was working at the time of the accident was about 44 inches per yard, in favour of the full tub, necessitating the use of lockers for a distance of 9 yards. He had evidently been attempting to take a full tub down this gradient, without a locker, and been in front of it and was overpowered and crushed between it and a prop.
BUDDLE Edward - 1906 - 17/11/1906 – Age 72 - Grassmore No.4 Dataller. Grassmore Co., Derbyshire. While repairing a brattice cloth, a horse drawing two empty tubs came through and knocked him down. He received injuries, but died of heart disease. The coroner's jury returned their verdict that ‘deceased’s death was caused by the accident, and that the accident was the primary cause of death.’ He died on 13th July, 1907.
BUDGE William Henry - 1905 - 21/11/1905 – Age 33 - Cramlington Hewer. Cramlington Coal Co. Ltd., Northumberland. Died November 22. Deceased worked alone in a bord, 6 yards wide in the Main Coal seam, made up of top coal 3 feet 6 inches band 8 inches and bottom coal 1 foot 7 inches. He had, in accordance with the usual practice, worked forward on the bottom coal and band nearly across the place but the right nook was back. He had two or three short props under the top coal while working under it, these he had withdrawn after drilling a shot hole in the top coal on the left side and nicking out the right side. The shot failed to bring down all the top coal leaving some hanging coal next the nicking. A fast shot had been fired in the bottom coal under this but when it was fired could not be ascertained. Deceased appears to have been working near this shot as his pick was there and he was found pinned against the bottom coal by a mass of the top coal 4 and 2.5 feet 10 inches thick which had fallen away from ordinary cleats and a parting above leaving 8 inches of coal sticking to the roof. It was suggested that fast shots are forbidden, deceased might have been engaged hewing away any evidence of it. In any case great want of caution was displayed in working near overhanging top coal shaken by a shot. The Inspectors reported, accidental, no blame attached anyone.
BUFFEN James - 1903 - 08/12/1903 – Age 30 - Saltwell No.37 Night overman. Earl of Dudley Staffordshire. While firing three shots of gunpowder together by safety fuse he returned after only two of the shots had exploded. The third shot exploded while he was standing under it and the coal fell on him and killed him. He and two other men with him thought they had heard three explosions.
BUILTH George - 1907 - 17/12/1907 – Age 25 - Crigglestone Labourer. Crigglestone Collieries Ltd., Yorkshire. A coal hopper for the coke ovens being erected for the Otto Hilgenslock Coke Oven Co., was in course of construction. The deceased had assisted to pull a long plank to a height of 20 feet and was standing under it while two men above were handling it. They foolishly dragged it across another two planks which formed a temporary scaffold and caused one, 10 feet in length, to fall and it struck the deceased on his chest and fatally injured him. This plank ought to have been properly secured in order to prevent an accident of this kind.
BULL Daniel - 1909 - 04/05/1909 – Age 56 - Broadfields Stallman. John Sheard, Derbyshire. Deceased was timbering the gate-end when a bar rolled off the prop supporting it and liberated the roof which fell on the deceased and an official of the mine who was there. These men were engaged at a depth of about 44 feet from the surface taking out the lining boards or sheetings of an old abandoned shaft, 4.5 feet square. The ground behind the boards was a very fine and dry sand. They appear to have stripped about 3.5 to 4 feet of the shaft when apparently a rush of sand occurred which knocked out some of the boards higher up and covered them and smothered them immediately. They called to be pulled up just as the rush commenced, but the evidence at the inquest was that they only called once when it was too late. They do not appear to have had previous experience in this immediate work and a mistake was made in that at least one man experienced at it did not go down, as one or more of the gang of five men had bad experience, but they decided among themselves who should go down and the authorities anticipated no danger. 2 killed.

BURNS John - 1904 - 05/09/1904 – Age 15 - Hebburn Driver. Wallsend & Hebburn Coal Co Ltd., Co.Durham. The engineman, of an underground haulage engine, working on the main tail rope system took the set of 80 empty tubs which had been detained by a signal outside the landing, or as termed, rapped back until the landing was sufficiently cleared of empty tubs, too far, and caused it to collide with four empty tubs left from the previous set, driving them forward upon the deceased, who was coming into the landing sitting on the limbers of a pony which was drawing two full tubs. The tail rope passes under the rail the inbye, the end of the landing to the turn sheave and one of these rails was torn up and a prop displaced before the set came to a standstill. While the set was coming into the landing there depressed the lever of a rapper released it when the set had come far enough and this caused a hammer to fall at a point nearer the shaft from where another lad signalled by another wire to the engineman. The engineman had in addition marked on his rope to guide him but there was no indicator on the engine to show the position of the set. The signals were given in the usual way but the engineman, although he elected not to give evidence at the inquest, stated to me he received a signal to stop. The Jury advised that a reliable indicator be attached to engines and effect had been given to this recommendation. The Local Inspectors in their report simply recited some of the facts as they observed them.

BURNS Patrick - 1908 - 01/02/1908 – Age 46 - Calder, Sinking Bricklayer. George Hargreaves and Co., Lancashire. In descending the shaft, in a small tub attached to a rope by two chains only. The engineman in lowering went farther than he ought to have done before retarding, he applied the break so as to set up oscillation, the tub being attached by only two hooks tilted. The tub collided with a beam with such force as to leave the deep cut impression on a wood beam, and all the men were thrown out. 4 killed.
BURNS Peter - 1908 - 20/07/1908 – Age 37 - Charlaw Hewer. Charlaw and Sacriston Collieries Co. Ltd., Co.Durham. He, with others, came to the shaft at the Busty Seam Level to ride. The onsetter, at this level, signalled that he wanted the cage by giving - the three-and a-half raps. The winding engineman, getting this signal, put the cage to the Busty Seam, and the onsetter, having the cage there, then, according to his own statement, rapped three raps, which was the signal that men were going to get into the cage. Having given that signal, the onsetter then told the men to get into the cage. One man got in but as Burns was getting in, the cage was drawn away and his leg was broken, a compound fracture. Blood-poisoning set in, and he died ten days afterwards. Instead of the cage being drawn away it should, supposing the signals to have been properly made, have remained at rest until the onsetter rapped one, then the engineman would wind it to the surface. The onsetter's statement is supported by the evidence of three other persons. The winding engineman acknowledged that he received the first signal - the three-and a-half raps - but he stated that the next signal he received was one of four raps, which meant that he had to take the cage to the Hutton Seam. He acted on this signal in ignorance of the fact that men were getting into the cage at the Busty Seam Level and Burns was caught. The statement of the winding engineman is supported by the evidence of one other person, and that of two others suggests there was some confusion of the raps. I think signals in reply should always be given when men are going to get into a cage to descend or ascend as the case may be.

The two deceased persons were descending the shaft, in the cage which collided with the ascending cage, owing to the fact that three bars of pitwood had been sent down in that cage to the bottom of shaft and had not been taken out at the mouthing of the Five Feet Seam, which was a landing about sixty yards from pit bottom and was the only mouthing where coal was being raised from. The timber in the ascending cage caught the descending cage and the deceased persons fell to the pit bottom and were killed. BURT Joseph - 1904 - 05/03/1904 – Age 27 - Dean Lane Collier. Bedminster Easton, Kingswood & Parkfield Collieries Co. Ltd., Somerset. A fall of roof where it was soft and ‘neash’ for a yard or more over the coal and in a place which seemed well secured. It is almost a wonder that more accidents have not occurred in the place which once more exemplifies the statement that if the same care were exercised by the men in places where the roof appears good as they do where they see it is really bad there would be many less accidents than do occur.
BURTON Arthur - 1907 - 26/03/1907 – Age 36 - Shirebrook Loader. Shirebrook Colliery Co., Derbyshire. The descending cage got fast in one of the conductor rods, causing the cage to tilt up and the three men fell down, the shaft. Two others were seriously injured. 3 killed.

His mates had left, and another man thinking no one was there, lowered another wagon by gravity. As soon as this closed on the standing one, a scream was heard and deceased was found fatally crushed between the buffers. He must have stepped between the wagons as they were closing.
BURTON George - 1908 - 16/12/1908 – Age 47 - Brandon Hewer. Strakers and Love, Co.Durham. This man and his marrow were hewing in a headways lift, the last of the pillar, five yards wide, when a large frame of post stone fell away from the roof, just as they were in the act of setting a prop. The stone fell away from a slip in the roof which had evidently just been exposed by the advancing face. The timbering distance was 6 feet, and the place was not well timbered. The men were, however, just about to put up timber when the accident occurred, and the deputy said that when he examined the place, about two hours prior to the accident, the place was then properly timbered.
BURTON J. - 1900 - 16/10/1900 – Age 55 - Garforth Byeworker. Garforth Coal Co., Yorkshire. Fall of roof.

The two men were seriously to blame which was also bound to be this case by this Coroner’s jury.
BURTON Joseph - 1908 - 28/10/1908 – Age 33 - Cadeby Main Clipper. Denaby and Cadeby Main Collieries Ltd., Yorkshire. The deceased was attaching full tubs to an endless rope on a road rising at the rate of 1 in 7, and either, on account of his not having screwed the clip tight enough, or because he had got the clip on to a splice in the rope, two tubs filled with dirt, after going 30 yards and ran back. He appeared to have been attempting to get into a refuge hole when he was caught by the tubs which dragged him about nine yards until they ran into a "jack catch" and tipped. He was so seriously injured that he died before being taken out of the pit. The clip was in working order and was, after the accident occurred, used on the same road and rope with three full tubs, and they were safely taken to the top of the plane, although the gradient on the way increased to 1 in 5. There was only one splice in the rope, where the two ends were joined together, and at that place the diameter of the rope was increased from 7/8 inches to 1 inch. Between the jack catch the tubs were caught by and the next one up the haulage road there was a distance of 90 yards. On a gradient of 1 in 7 they should have been one at least every 20 yards.
