SKYM David - 1901 - 24/05/1901 - Universal Haulier, Universal Steam Coal Co. Ltd., Glamorgan. This explosion was specially reported upon by W Galloway, Cd 947. 81 killed.

SLACK Thomas - 1900 - 24/01/1900 – Age 40 - Hall End Stallman, Morris and Shaw, Warwickshire. Fall of roof. Deceased appeared to have been setting a prop, just prior to him leaving work for the day, when a piece of coal came off the face, and a heavy fall of roof with it.

SLACK Thomas - 1908 - 24/01/1908 – Age 30 - Clifton Haulageman, Clifton Colliery Co., Nottinghamshire. He was examining the tension wheels of the endless rope haulage, and although the brakes had been put on the pulleys moved and crushed him.

SLACK Thomas William - 1900 - 04/12/1900 – Age 27 - Hopton Wood Limestone Quarry Quarryman, Hopton Wood Stone Co., Derbyshire. Deceased fell down the Quarry face, a distance of 24 feet, while working with an iron bar getting stone. He was probably using the bar and slipped while doing so. He died the next day.

SLANEY Harry - 1909 - 18/01/1909 – Age 15 - Mansfield Horse Driver, Bolsover Colliery Co. Ltd., Derbyshire. He was turning his horse round in a pass-bye, when the slowly moving tubs struck the limbers and fastened the deceased against other tubs.

SLATER John Thomas - 1909 - 18/01/1909 – Age 32 - Coneygre No.2 Pikeman, Blewitt and Bradley, Worcestershire. Deceased was working in a stall road, 9 feet wide, having the old "shut" on one side the road. He was knocking out a prop on the gob side with a "bunter” with a view to dropping some 3 feet of coal above when, on the prop being released, the road collapsed, the timber being reeled out and deceased buried. He was dead when he was got out. Another pikeman working with deceased was caught by the fall and injured. A slip was revealed along the rib side which therefore caused the place to have two loose sides. The drawing of the timber, which was very tight, showing "weight" would disturb the roof and led to the sudden collapse.

SLATER Samuel - 1908 - 11/05/1908 – Age 49 - Plumptre Stallman, Butterley Co. Ltd., Nottinghamshire. Fall of roof. Deceased was in the act of getting down a slab of coal when a piece of the roof fell and fatally injured him.

SLATER T. - 1903 - 25/08/1903 – Age 64 - Don Pedro, Collier, H Briggs Son and Co. Ltd., Yorkshire. Died 26th Aug. Fatally injured by a flake of coal which was taken from the face, coming sooner than anticipated.

SLATER Thomas - 1905 - 22/02/1905 – Age 36 - Flappit Spring, Sandstone Quarry Barer, Joseph Clayton Ltd., Yorkshire. Along with another three men they were removing baring dirt in the Quarry, which had been loosened by a charge of gelignite on the 17th, when one of them struck some unexploded and frozen cartridges which exploded and killed these two and injured two others. 2 killed.

SLATHERTHWAITE William - 1908 - 14/04/1908 – Age 40 - Aldwarke Main Drifter, John Brown and Co. Ltd., Yorkshire. He was assisting to set a prop under the end of a steel girder, supporting the roof of a road, and while doing so was standing under a wooden bar 6 feet away. As soon as the prop under the girder was hit, to tighten it, a fall of roof occurred, breaking the bar under which he was standing and displacing the girder.

SLAVIN Francis - 1909 - 14/09/1909 – Age 16 - Greenfield No.1 Miner, Archibald Russell Ltd., Lanarkshire. Fall of side at road-head from an unseen lype.

SLEE James Edward - 1909 - 11/02/1909 - Baptist Well Quarry Quarryman, Pye Parkinson and Co. Ltd., Glamorgan. While using an iron bar getting stone, the bar slipped and caused him to twist round and wrench himself. A post mortem was held, and the doctor stated that he found the stomach dilated and bruised. Death attributed to syncope from gastric haemorrhage.

SLEIGH Charles - 1909 - 08/02/1909 – Age 38 - Shotton Hewer, Horden Collieries Ltd., Co.Durham. He was killed by a stone, weighing about half a ton, which fell directly on to him as he was hewing. There were slips in three sides of the stone but these would not be visible prior to the stone falling.

SLESSOR James - 1901 - 28/05/1901 – Age 45 - Ben Cruchan Granite Quarry Quarryman, Ben Cruchan Granite Quarries Ltd., Argyll. When jumping up upon a moving loco., in a siding besides the main line, he was crushed against a wagon on the adjoining line of rails.

SLINN John – 1908 – 28/05/1908 – Age 42 – Seaton Delaval Hewer, Seaton Delaval Coal Co. Ltd., Northumberland. Fall of side stone at working place.

SLOAN Hugh – 1901 – 06/05/1901 – Age 25 – Monkland Miner, James Dunlop and Co. (1900) Ltd., Lanarkshire. Fall from roof at face of longwall in the upper Drumgray seam. The fall was 20 feet long, 3 feet wide, and 1 foot thick and came away from a crack and swung out some timber.

SLOAN Hugh jnr. – 1909 – 03/08/1909 – Age 16 – Little Mill No.2 Drawer, Coylton Coal Co., Ayrshire. Fall of roof in working place. The prescribed distance between props was exceeded. (Another man injured)

SLOAN John – 1902 – 16/01/1902 – Age 40 – Newton No.2 Miner, James Dunlop and Co. Ltd., Lanarkshire. Fall of roof in working place.

SLOAN John – 1909 – 06/10/1909 – Age 36 – Lightshaw No.1 Miner, William Baird and Co. Ltd., Ayrshire. Fall of coal while taking it down.

SLOANE Joseph – 1907 – 25/01/1907 – Age 21 – Polton Miner, Lothian Coal Co. Ltd., Edinburgh. Deceased was holing his place and had just finished when the roof suddenly collapsed and fell upon him; the holing was on the top of the seam.

SLOPER Thomas – 1903 – 10/02/1903 – Age 22 – Cwm Pointsman, Crawshay Bros., Cyfartha Ltd., Glamorgan. Deceased fell as he was getting off a locomotive while it was in motion and he was run over. It was his first day at the work and I expect he was anxious to do the work expeditiously.

SLOYAN Patrick – 1908 – 18/08/1908 – Age 29 – Maypole Contractor, Moss Hall Co., Wigan, Lancashire. Explosion of firedamp and coal dust. 75 killed.

SLUMLEY Charles – 1909 – 27/02/1909 – Age 20 – Waun Llwyd Collier, Ebbw Vale Steel Iron and Coal Co. Ltd., Monmouth. Fall of side from a Slant, as he was clearing rubbish from a Barry work, knocked him down, injuring his back against a stone on the floor. He was sent to the Hospital and it was found that his spine was partially fractured. He died on the 5th of April from "Cystitis" and general blood poisoning.

SMAILES J – 1903 – 17/11/1903 – Age 42 – Lofthouse Collier, Lofthouse Colliery Co. Ltd., Yorkshire. Fall of roof in a working place. Cause heavy bump and a slip.

SMAILES T. – 1903 – 18/09/1903 – Age 23 – Oulton Sandstone Quarry Crane Driver, George Armitage and Sons Ltd., Yorkshire. Died 19th September. He had lifted a stone with the crane and the machinery out of gear before he got the jib round. He probably did not put on the break for four driving gears were whirled round, so rapidly that they burst and Smales was fatally injured. Pieces of the wheel were hurled 100 yards.

SMALE Joseph – 1903 – 17/02/1903 – Age 55 – Broughton Moor, Slate Miner, F Wickwar,  Lancashire. Deceased, who was an experienced iron ore miner but had only worked at this mine for three weeks was one of a gang of ten iron ore miners who had taken a contract. Only one of the gang had had experience in slate mines. A chamber 16 feet high by 18 feet wide had been opened out 70 yards from the entrance of a level, 6 feet square, the level face being 10 yards further in. A shot was fired in the chamber some distance from where the fall took place, about an hour before the accident, the deceased and his mate and after it the roof and sides were sounded and considered safe but several tons of stuff fell from the level side opposite the chamber and killed deceased.

SMALLEY George – 1903 – 20/01/1903 – Age 24 – White Rock, Clay Quarry Quarryman, AF Houfton, Nottinghamshire. Deceased and another man were loading clay near the face of the Quarry when a portion of sand overburden rolled over and killed deceased. The other man was injured.

SMALLEY Henry – 1900 – 28/03/1900 – Age 31 – Moss No.6 Collier, Pearson and Knowles Coal and Iron Co. Ltd., Wigan, Lancashire. Inspection made and inquest attended. He was engaged filling a tub of coal, near the face of the roadway when a piece of roof coal fell upon him from between two slips running parallel with the roadway. The road was propped on each side but not barred. He succumbed to the injuries received at the infirmary the following day.

SMALLEY Reuben - 1903 - 30/06/1903 – Age 59 - Ashton Moss Collier, New Moss Colliery Ltd., Lancashire. Fall of roof in working place from a 12-inch fault; recklessly knocked out a prop leaving the roof not supported which was required by the timbering direction. (Average roof)

SMALLWOOD Cyril - 1904 - 19/08/1904 – Age 25 - Alma No.4 Dayman, Alma Colliery Co., Derbyshire. Fall of roof. Deceased was wedging down the top coal and when it fell a portion of the roof fell from a pothole and killed him.

SMART J. - 1900 - 14/03/1900 - Allerton Bywater Collier, Silkstone and Haigh Moor Coal Co., Yorkshire. Fall of roof.

SMART James - 1905 - 20/01/1905 – Age 16 - Bold No.3 Haulage Hand, Collins Green Colliery Co., St. Helens, Lancashire. Inspection made and inquest attended. A large fall of roof occurred at a junction of roads knocking down the bars and covering timber, which seem to have been reeled out without breaking from some cause, fatally injuring them. 2 killed.

SMART James Berwick - 1901 - 25/11/1901 – Age 46 - Balognie Roadsman, CB Balfour, Fife. Deceased and three other workmen were repairing a main horse road, usually on the usual idle day, when a large fall took place, off the side and crashed through the timbers completely burying him.

SMART John - 1909 - 03/03/1909 – Age 23 - Netherton No.20 Cager, Mrs GH Dunn, Worcestershire. Fell part way down the shaft.

SMART WB - 1900 - 23/08/1900 – Age 31 - Aldridge Stallman, Aldridge Colliery Co. Ltd., Staffordshire. Fall from working face when wedging coal.

SMEDLEY Charles - 1907 - 30/10/1907 – Age 21 - Mansfield Loader, Bolsover Colliery Co., Nottinghamshire. Some full tubs had been left in the gateroad, when they ran back into the gate-end and striking another tub standing there, injured the deceased. He died the same day.

SMEDLEY George - 1909 - 19/01/1909 – Age 63 - Swanwick Engine Driver, CRP Morewood, Derbyshire. Deceased was crossing the sidings when he was run over by some empty railway wagons. A bridge goes over the rails, and deceased should have used it.

SMEDLEY Thomas - 1909 - 04/12/1909 – Age 49 - Frickley Collier, Carlton Main Colliery Co. Ltd., Yorkshire. A bump burst a large slab of coal on the face. Smedley was pinned between the coal face prop and killed on the spot.

SMEDLEY William - 1906 - 05/11/1906 – Age 46 - Pinxton No.1 Stallman, Pinxton Collieries Ltd., Derbyshire. Fall of roof. Deceased was walking along the coal face of his stall, when a fall of roof killed him.

SMEDLEY Windsor - 1902 - 13/09/1902 – Age 24 - Aberaman Fitter, Powell Duffryn Steam Coal Co. Ltd., Glamorgan. Deceased was raising, by means of a steam crab, a rope in the shaft when his neck muffler got twisted round the intermediate shaft and strangled him. He must have been leaning over the revolving shaft. The mechanic who was watching the rope coiling on the opposite side of the drum ran and shut off steam as soon as he became aware of the accident but too late to save him.

SMELLIE Robert - 1909 - 24/02/1909 – Age 36 - Hopetoun No.44 Oil Shale Oversman, Youngs Paraffin Light and Mineral Oil Co. Ltd., Linlithgow. While an empty rake was descending an ingoing eye, one tub uncoupled and ran down against three others, resting on a prop on centre of roadway causing these tubs to run away; deceased was doing some repairs to a pump on the roadside, and for safety he crossed over to a branch road and just then the runaway tub left the rails and crushed him against a wooden pillar.

SMELT William - 1900 - 20/08/1900 – Age 44 - Westerton Keeper, Bolckow Vaughan and Co. Ltd., Co.Durham. The rope which hauls the tub up to apparatus on surface broke and the tub ran away and caught deceased at the bottom of the incline and killed him.

SMETHURST William - 1901 - 30/09/1901 – Age 26 - Blackbrook Daywageman, Blackbrook Colliery Co., St. Helens, Lancashire. Inspection made and inquest attended. He was engaged clearing away the holing dirt displaced by an electrical heading machine, when his foot slipped on a wet stone floor and his leg was caught by the revolving cutters and he was drawn in and fatally injured. He expired at the Haydock Hospital the same night.

SMILES John - 1903 - 23/02/1903 – Age 23 - Pelton Hewer, Owners of Pelton Colliery, Co.Durham. A fall of stone with glassy slips all round it from the roof killed him.

SMISSEN Richard - 1909 - 14/01/1909 – Age 43 - Tilmanstone Sinker, East Kent Colliery Co. Ltd., Kent. The winding engine man overwound the water barrel, which becoming detached from the rope, owing to breakage of the pin of the D link, it fell down the shaft and caused the death of these three men who were on a scaffold to steady the barrel, about 310 yards from the surface and 5 or 6 yards from the bottom. 3 killed.

SMITH A. - 1903 - 13/01/1903 – Age 37 - Abbey, Sandstone Quarry Quarryman, George Vinty and Bros., Yorkshire. Fall of clay overburden of a sandstone Quarry. Cause frost. 2 killed.

SMITH Alexander - 1901 - 26/08/1901 – Age 48 - Donibristle Oncostman, Donibristle Colliery Co., Fife. Inflow of moss. 9 killed.

SMITH Alexander - 1908 - 19/12/1908 – Age 41 - Michael Fireman, Wemyss Coal Co. Ltd., Fife. Deceased was securing the roof of a main haulage dook which was showing signs of collapsing, when a large fall occurred burying him under it.

SMITH Alfred - 1902 - 20/11/1902 – Age 19 - Fryston Onsetter, Wheldale Coal Co. Ltd., Yorkshire. He was foolishly crossing the shaft bottom when he was crushed by the descending cage.

SMITH Alfred - 1904 - 21/09/1904 – Age 32 - Brownhills No.3 Loader, W Harrison Ltd., Staffordshire. Fall of side (coal) at longwall face Deep seam. A rate of coal 9 feet long and 5.5 feet high, 2 feet thick at top and wedge-shaped towards the bottom fell over and crushed him against a tree.

SMITH Arthur - 1900 - 27/01/1900 – Age 40 - Barkham Road Sand Quarry Carter, AF Walters, Berkshire. Deceased and his son were undercutting a face when it rolled over and killed deceased and seriously injuring his son. The greatest depth of the Quarry was 17 feet 3 inches so that it was not under the Act.

SMITH Charles - 1906 - 12/03/1906 – Age 62 - Grassmoor No.8 Dataller, Grassmoor Co., Derbyshire. Fall of roof. Deceased was setting a bar to support the roof when a large fall occurred, coming away at a fault slip which ran parallel to the roadway and was well known to deceased.

SMITH Charles - 1907 - 05/09/1907 – Age 15 - Denaby Main Pony Driver, Denaby and Cadeby Main Collieries Ltd., Yorkshire. He was found underneath the first full tub of a set of eight at a place where he had to put lockers in the wheels.

SMITH Charles - 1907 - 25/03/1907 – Age 30 - Blantyre Ferme Drawer, AG Moore and Co., Lanarkshire. While illegally drawing in front of his hutch, going down an incline, the snibble broke and he was knocked down by the hutch and crushed against a prop.

SMITH Charles - 1907 - 28/02/1907 – Age 24 - Balognie Labourer, Charles B Balfour, Fife. Deceased was inside a cylindrical water softening apparatus, cleaning out the wood fibre which lay to a depth of six feet below the top of the tank, when the iron plate supporting the fibre gave way and he was precipitated into some hot water below which had not been drawn off. This plate was supported by two bolts which appeared to be insufficient to sustain this weight.

SMITH Charles H. - 1903 - 29/01/1903 – Age 31 - Shildon Shotfirer, Shildon Coal Co. Ltd., Co.Durham. He was hit on the thigh by a piece of stone which caused a wound about the size of a two shilling piece. Tetanus set in and he died 28th.

SMITH David - 1905 - 19/06/1905 – Age 53 - Tillery Pumpman, Powells Tillery Steam Coal Co. Ltd., Monmouth. He was engaged attending to a pump at an offset in the shaft and stepping from the landing into the cage he fell to the bottom, it is alleged in consequence of striking his head against the top hoop of the cage.

SMITH David - 1907 - 27/03/1907 – Age 30 - Burn Moor Master Shifter, Lambton Collieries Ltd., Co.Durham. He was electrocuted by a leakage of current from the trailing cable of a coal cutter.

SMITH E.A.T. - 1909 - 09/02/1909 – Age 17 - Llanhilleth Collier, Partridge Jones and Co. Ltd., Monmouth. Fall of roof at the face from a glassy parting more or less all around and on the top of it, which owing to a scale of coal under the stone could not be seen beforehand. Size of stone, 4.5 feet by 2.5 feet by 9 inches thick.

SMITH Edward - 1900 - 15/01/1900 – Age 36 - Moss No.4 Dataller, Pearson and Knowles Coal and Iron Co. Ltd., Wigan, Lancashire. Inspection made and inquest attended. Deceased was engaged roofing down and appears to have cut one side a little distance and there being a slip on the other side notwithstanding this he appears to have gone under the stone before getting it down when it fell upon him.

SMITH Edward - 1903 - 19/05/1903 – Age 13 - Llwydgoed Collier Boy, Llwydgoed Collieries Ltd., Glamorgan. Fall on road cliff, 4 feet 3 inches by 27 inches by 6 inches thick, fell off the side as he was passing. The arms from between which it fell were only two feet apart.

SMITH Edward - 1904 - 08/03/1904 – Age 54 - Griff Clara Banksman, Griff Colliery Co., Warwickshire. Crushed by two loaded railway wagons while attempting to pass through an opening between them and while other wagons were being lowered into them.

SMITH Edward - 1906 - 06/03/1906 – Age 36 - Llay Hall Contractor, Exors of ES Clark, Denbigh. Inspection made and inquest attended. He was wedging a piece of coal, off the face, when some clod and roof fell from a slip running with the face, between the last row of props and the face. He succumbed to the injuries on the 8th instant.

SMITH Edward Thomas - 1904 - 19/08/1904 – Age 16 - Maindy Haulier, Ocean Coal Co. Ltd., Glamorgan. Found dead under a loaded tram. It is supposed that he was riding on the gun and fell off.

SMITH Edwin - 1906 - 19/02/1906 – Age 17 - Dinnington, Granville Assistant Taker Off, Lilleshall Coal Co., Shropshire. Deceased was killed by falling from a landing at the Double Coal to the shaft bottom at the Clod Coal, a distance of 50 yards. How he came to do so is not known, for after the occurrence the gates at the inset were found closed and latched.

SMITH Enoch - 1900 - 19/11/1900 – Age 58 - Mosley Common Trencherbone Collier, Bridgewater Trustees, Lancashire. Fall of roof in working place from pronounced fault and slips, he delayed setting a prop until coal filled. He was filling the second wagon after knowing a prop was required. (Bad roof).

SMITH Ernest Blair - 1909 - 16/02/1909 – Age 28 - West Stanley, Owners of West Stanley Colliery, Co.Durham. Explosion of gas. 168 killed. 34 High Street Stanley.

SMITH Ernest Edward - 1905 - 06/11/1905 – Age 19 - Houghton Tub Attendant, Lambton Collieries Ltd., Co.Durham. He was caught between the side of the drum and the shaft carriage while taking "beeswing" chain off, and received injuries from which he died the same day.

SMITH F.H. - 1904 - 21/03/1904 – Age 17 - Apedale, Burley Shifter, Midland Coal Coke and Iron Co. Ltd., Staffordshire. Fall of roof (bass) in an air level in the 7 feet Banbury seam. Inclination of seam 1 in 2.7. Potts and Smith were repairing the level and they appeared to have gone a few yards in advance of the repairs probably for examination when a piece of overhanging bass fell from the roof, on the rise side of the level and caught them. The place was remote from other workings, and the exact cause of the accident was unknown. Potts was dead when found, and Smith died the same day. 2 killed.

SMITH Frank - 1903 - 13/12/1903 – Age 40 - Maindy Trimmer, Ocean Coal Co. Ltd., Glamorgan. Deceased, when coming out from between wagons under the screens was caught and run over by a passing train of empty wagons which were being let down at a considerable speed by a hauling engine.

SMITH Fred - 1904 - 15/07/1904 – Age 28 - Mitchell Main Shunter, Mitchell Main Colliery Co. Ltd., Yorkshire. It was his duty to feed the screens with empty waggons and three were standing in a siding which has a gradient of about 0.25 inch per yard towards the screens; one was about 18 inches in front of the other two and he foolishly went between the buffers to push the waggon to the screens. While in that position two other waggons brought down by a horse ran into the two standing waggons and deceased was crushed fatally. The driver of the horse could not see him and went away without knowing that an accident had happened. There was no need for him to go between the buffers at all as the waggon could be moved with a stick he had with him and proper pinch bars are provided.

SMITH Fred - 1907 - 01/05/1907 – Age 16 - Warsop Main Rope Boy, Staveley Coal and Iron Co., Nottinghamshire. He was fatally crushed between a full set and an empty set of eight tubs each passing along a roadway worked by endless rope. The youth was working overtime when the accident occurred.

SMITH Frederick - 1903 - 08/10/1903 – Age 23 - Merthyr Vale No.1 Collier, Nixons Navigation Co. Ltd., Glamorgan. Fall of roof at face coal (roof) 3 feet by 21 feet by 11 feet thick, fell from between two slants and between a prop and the face. Nine feet seam.

SMITH Frederick Charles - 1902 - 09/08/1902 – Age 13 - Baddesley Rope Boy, Baddesley Colliery Trustees, Warwickshire. Knocked down by a train of full tubs which had been detached from the hauling rope and running into the pit bottom. The deceased was probably trying to remove some timber out of the way of the full train when he was run over.

SMITH George - 1900 - 25/06/1900 – Age 40 - Mountsorrel Main Granite Quarry Labourer, Mountsorrel Granite Co., Leicestershire. Deceased was engaged in removing two railway waggons when he slipped on the loose granite lying between the two roads and fell under the moving waggon. He died on the 4th July from lockjaw following the injuries he received.

SMITH George - 1902 - 04/06/1902 – Age 57 - Hambledon, Limestone Quarry Quarryman, John Green and Co., Yorkshire. A stone fell from the face of the Quarry, 80 feet above him, and rebounding from the side killed him on the spot.

SMITH George - 1902 - 10/10/1902 – Age 22 - Cefn Slip Collier, Bryndu & Port Talbot Collieries Co. Ltd., Glamorgan. Fall of side at face slip of coal fell suddenly from the upper side in a long cut one sprag set. Cribbwr seam. 2 killed.

SMITH George - 1903 - 13/01/1903 – Age 44 - Birley, South Dataller, Sheffield Coal Co., Derbyshire. Fall of roof. Two steel bars supporting the roof of a main level had fallen and the two deceased were setting a centre prop under the covering timber which appears to have taken the weight off the adjacent bars causing them to fall. The debris above the covering timber fell and killed the two deceased. 2 killed.

SMITH George - 1903 - 24/12/1903 – Age 32 - South Skelton Deputy, Bolckow Vaughan and Co. Ltd., North Yorkshire. Thigh fractured and back injured by fall of stone from face. Died same day.

SMITH George - 1904 - 08/12/1904 – Age 40 - Gelderfield Road, Brickearth Quarry Clay getter, Clayton and Speights, Yorkshire. He was walking round a large mass of material to ascertain the condition of the face where nothing had been touched for some time and was turning round when a bucketful of shale fell from the face, some 40 feet up, and caught him on the head, injuring him so severely that he died the same day. The fall was a very small one, it was due to the sun on the face after a frost.

SMITH George - 1906 - 05/03/1906 – Age 41 - Fir Grove Gravel Quarry Labourer, T Patterson and Son, Surrey. They were working in a trench about 6 feet deep at the bottom of the face of sand and gravel, when a quantity slipped out and buried them. 2 killed.

SMITH George - 1906 - 14/10/1906 – Age 46 - Wingate Sump Emptier, Owners of Wingate Grange Colliery Ltd., Co.Durham. Explosion of coal dust result of shot firing. 25 killed.

SMITH George - 1907 - 11/09/1907 – Age 39 - Brackenhill, Sandstone Quarry Quarryman, Webster and Co., Yorkshire. He was working on a ledge 18 feet from the top of the Quarry and 60 feet above the bottom of it when he either slipped or was seized with a fit of dizziness and fell to the bottom of the Quarry and was so seriously injured that he died within half an hour.

SMITH George - 1908 - 09/07/1908 – Age 25 - Chapel Chain Runner, Chapel Coal Co. Ltd., Lanarkshire. Deceased was riding on the fourth tub of his rake up a dook 1 in 8. The tub he was on got off the rails and he was jumping into a manhole but did not make it and was caught and dragged 18 feet. He was injured, 9th July 1908, and died 25th Mar 1909.

SMITH George - 1909 - 27/09/1909 – Age 38 - Soothill Wood Miner, Soothill Wood Colliery Co. Ltd., Yorkshire. He was working in the face, close to a fault, when a large stone fell from between the timber. The fall disclosed converging slips.

SMITH George R. - 1907 - 11/09/1907 – Age 23 - Dawdon Sinker, Londonderry Collieries Ltd., Co.Durham. They were thrown from a cradle, on which they were standing, to the bottom of the pit and killed. 3 killed.

SMITH Harold - 1909 - 30/06/1909 – Age 13 - Hodroyd Pony driver, New Monckton Collieries Ltd., Yorkshire. He was passing along a roadway when a large stone fell from the roof and killed him. There was a prop at the edge of the stone which fell. The fall was caused by a bump.

SMITH Harry - 1905 - 22/11/1905 – Age 47 - Crymlyn Quarry Quarryman, Topham Jones and Railton, Glamorgan. While working on the face of the Quarry, probably barring some stones down, he was either struck by a stone from above and knocked off, or had over-reached himself in the act of barring and fallen off. Died in Swansea Hospital the following day while undergoing amputation.

SMITH Henry - 1900 - 29/01/1900 – Age 24 - Woodside, Shipley Dayman, EM Mundy, Derbyshire. While using a ringer or iron bar to the coal face he appears to have slipped in such a way that the ringer injured his body. He died on the 31st.

SMITH Henry - 1903 - 06/01/1903 – Age 51 - Eston Miner, Bolckow Vaughan and Co. Ltd., North Yorkshire. A piece of sulphur stone fell from the roof and so severely injured him that he died the following day.

SMITH Henry - 1905 - 20/11/1905 – Age 29 - Clifton Stallman, Clifton Colliery Co., Nottinghamshire. Fall of coal. deceased was taking a sprag out, and when the coal fell he stumbled against a prop and fell under the coal.

SMITH Henry - 1906 - 20/02/1906 – Age 50 - Southfield Stone Quarry Labourer, Mrs Ruth Chadwick, Lancashire. He was seen to stoop to pick up his spade; apparently the stooping affected him, for he stood up and, reeling round with his arms held up he fell over the ledge, 2 feet below his head struck the top of a waggon, thence he fell 5 feet to the ground. Doctor on post-mortem found congestion of the brain, but whether this arose naturally or from the fall could not be proved. There was no injury to the brain. Ledge 5 feet wide.

SMITH Henry - 1907 - 17/10/1907 – Age 61 Waun Llwyd Brakesman Ebbw Vale Steel Iron and Coal Co. Ltd., Monmouth. When lowering trucks to the screens by the brakes he was crushed between the truck and the retaining wall the space not being more than 12 ins. He was using the brake on the second last waggon.

SMITH Henry - 1908 - 14/08/1908 – Age 15 - Elliot Collier, Powell Duffryn Steam Coal Co. Ltd., Monmouth. Fall of roof clod at the face from two slants, one of which was visible, but the other could not be seen as the coal had not been taken out from under it before the accident.

SMITH Henry - 1909 - 04/05/1909 – Age 48 - Kingsbury Header, Kingsbury Collieries Ltd., Warwickshire. Deceased and a loader were clearing up the results of a shot fired at the face of a stone-heading, when a fall of roof came from beyond the completed side of the heading and killed deceased.

SMITH Isaac - 1903 - 10/03/1903 – Age 48 - Linnyshaw Collier, Bridgewater Trustees,  Lancashire. Fall of roof from pot-hole in working place, 7 feet 6 inches beyond last prop, rule requires props to be every 6 feet, it ought to have been 4 feet. Died 14th March. (Bad roof).

SMITH J. - 1903 - 04/11/1903 – Age 19 - Car House Rope Rider, John Brown and Co. Ltd., Yorkshire. He was walking up a roadway when a large piece of rock fell from the roof and killed him. The fall disclosed a conglomerate or pebble parting.

SMITH James - 1900 - 06/08/1900 – Age 19 - Allanton Brakesman, William Barr and Sons, Lanarkshire. Deceased was running alongside some wagons to regulate their speed by the brake of the first one as they passed into a siding when by some means he stumbled and fell, one of the wagons passed over him. The brake on the wagon by which the speed was regulated was a patent one.

SMITH James - 1900 - 15/12/1900 – Age 38 - Rose Hill Trencherbone Shotlighter, S Scowcroft and Sons, Lancashire. Burned about lower part of body from ignition of a 3 ounce cartridge, Bulldog powder, probably protruding from his trouser pocket, it was ignited by a spark from a fuse. He had not put the powder canister into place. Usually fired by electricity. Battery out of order that day.

SMITH James - 1904 - 01/10/1904 – Age 26 - Leven Fife Miner, Fife Coal Co. Ltd., Fife. Deceased was engaged taking down a piece of stone, immediately above the bottom portion of the seam, when another piece fell and fractured his left leg. He died from his injury on 16th August, 1905.

SMITH James - 1904 - 04/06/1904 – Age 22 - Chanters Shunter, Fletcher Burrows and Co. Ltd., Lancashire. Crushed by waggons during shunting operations, the waggons were not sufficiently secured, crept away.

SMITH James - 1904 - 10/02/1904 – Age 26 - Whitehaven, Croft Trailer, Whitehaven Colliery Co. Ltd., Cumberland. Died August 29th. While travelling outbye along on an endless rope haulage road deceased was injured by the sets. The full sets consist of 12 tubs with a bogie in front the empty sets consisting of 24 tubs with a bogie at each end. The ends of the bogies are whitewashed to make their approach more conspicuous. The speed of the rope is three miles per hour. No person rides with the sets. Deceased was heard shouting by two men going inbye and they immediately signalled and the rope stopped. Deceased was found lying on the empty road opposite his second tub of a full set and the last bogie of an empty set was opposite the eleventh tub of the full set. The road was in fair order and provided with sufficient manholes, which, however, were not all on the same side. There was barely room for a person to stand either between the side of the road and the tubs or between the full and empty set. The height clear of the top of the tube in the immediate neighbourhood was not sufficient to allow anyone to ride safely on a loaded tub which, notwithstanding it is strictly prohibited is sometimes practised and men are fined in consequence. Deceased stated he was getting out of the way of one set when he was caught by the other.

SMITH James - 1904 - 11/05/1904 – Age 44 - Quarter No. 8 Brusher, United Collieries Ltd., Lanarkshire. Fall of roof and side in working face.

SMITH James - 1906 - 14/04/1906 – Age 28 - Saron Collier, Hills Plymouth Co. Ltd., Glamorgan. Fall of roof on road, cliff, 7 feet by 5 feet by 16 inches thick, fell as deceased was putting up a prop to carry an electric power cable. No.3 Rhondda seam.

SMITH James - 1907 - 02/11/1907 – Age 40 - Redding Coal Cutter, James Nimmo and Co. Ltd., Stirling. A brushing shot had been fired at the roadhead, which brought down the roof for some distance on one side of the wall; after the debris thus brought down, had been cleared, to allow the coal cutter to pass, cutting was again commenced and shortly after the roof fell upon deceased killing him instantly. The charge of the shot was too heavy and broke the roof across the face and the fall was due to this cause.

SMITH James - 1907 - 16/05/1907 – Age - 43 Kippsbyre Pitheadman, Strain Bros., Lanarkshire. When the cage arrived at the surface, deceased found the tub fast,, by reason of the coupling link having caught on the cage and he got round behind to relieve it, and shouted to the engineman to raise, but the latter took it to mean that all was right and began to lower, deceased was carried down the shaft and seriously injured. He succumbed to his injuries eight months after.

SMITH James William - 1904 - 09/02/1904 – Age 31 - Killingworth Lizzie Wagon Driver, Burradon and Coxlodge Coal Co. Ltd., Northumberland. Owing to a congestion of wagons deceased was using his horse to draw three empty chaldron wagons down a slight decline back on to the main line along which they had come when nearly far enough and at a point where two rails crossed he was in the act of unhooking the horse's chain when his foot was caught in the angle formed by the junction of the two rails and he was knocked down by the first wagon and it and the one following passed over his leg. The angle between the rails was not filled up, as is desirable, by a wood block. He was sent off to the Newcastle Infirmary but died on the road.

SMITH Jesse - 1909 - 02/04/1909 - King’s Dyke, Clay Quarry Quarryman, AW Itter, Cambridge. He was working on a ledge when a piece of clay, 28 lbs. in weight, fell from the face, about 15 feet above him and caught and killed him.

SMITH Job - 1908 - 24/11/1908 – Age 39 - Walsall Wood Shifter, Walsall Wood Colliery Co. Ltd., Staffordshire. A small quantity of dirt having fallen from the side in a drift rising 1 in 4 across the measures, which dip 1 in 9 deceased and another man were sent to clean up the road. The drift was secured by bars and girders which were set on props at one side and let into the rock at the other with lagging on the top, and at the place of the accident a space above the lagging had been partially filled up by a quantity of dirt. While working there two of the girders were suddenly reeled out and the lagging and dirt falling upon deceased knocked him down and buried him. He was got out with all speed and artificial respiration tried but without success.

SMITH John - 1901 - 11/03/1901 – Age 19 - Blair No.2 Ironstone Miner, William Baird and Co., Ltd., Ayrshire. Fall of roof at working face. Caused by want of props.

SMITH John - 1901 - 12/08/1901 – Age - 50 - Eston Ironstone Miner, Bolckow Vaughan and Co. Ltd., North Yorkshire. When breaking stone, to fill into a tub, a large piece of sulphur and ironstone fell away at a slip in the roof and killed him.

SMITH John - 1902 - 16/01/1902 – Age 35 - Cowdenbeath Miner, Fife Coal Co. Ltd., Fife. Deceased had been redding and timbering a fallen road in a longwall working when a large stone burst suddenly from the sandstone roof killing him instantaneously.

SMITH John - 1902 - 21/05/1902 – Age 16 - Dinnington Putter, Seaton Burn Coal Co. Ltd., Northumberland. Explosion of firedamp or coal dust.

SMITH John - 1902 - 22/12/1902 – Age 54 - Neepsend, Brick Earth Quarry Quarryman, Sheffield Brick Co. Ltd., Yorkshire. 800 tons of shale and stone slipped from the top of the face and some of it rolled over a previous fall at which Smith was working and killed him.

SMITH John - 1902 - 24/12/1902 – Age 28 - Breighmet Drawer, T and HT Scowcroft, Lancashire. Crushed by a tub whilst passing the bottom of the jig brow tunnel from the Doe Five Quarters Seam. A tub was pushed into the jig brow by attendant not coupled to a rope and no signal given.

SMITH John - 1903 - 13/01/1903 – Age 57 - Abbey, Sandstone Quarry Quarryman, George Vinty and Bros., Yorkshire. Fall of clay overburden of a sandstone Quarry. Cause frost. 2 killed.

SMITH John - 1903 - 23/12/1903 – Age 68 - Wilderness Stone Quarry Labourer, Forest of Dean Stone Firms Ltd., Gloucester. Fall or slide of a large mass of sandstone rock and rubbish from above where they were filling boxes for the crane. The bed of stone lies on a thin bed of marl dipping at a high angle, this in consequence of the wet weather caused the stone to slide away suddenly from joints which released it as the gutter, which had been only just been cut through at the bottom for the purpose of working this bed of stone was narrow, these men were unable to get away. It does not appear to have given any previous indication of being loose. 2 killed.

SMITH John - 1904 - 11/05/1905 – Age 17 - Elemore Putter, Hetton Coal Co. Ltd., Co.Durham. Found lying by the side of tub way unconscious with his skull fractured. Died 13th instant. It is probable that his head was crushed between the back of the tub and the frame of the canvas door or owing to the pony pushing back.

SMITH John - 1904 - 16/02/1904 – Age 46 - Cymmer Old Collier Insoles Ltd., Glamorgan. At face cliff, 8 feet by 2 feet by 14 inches thick, part of the rippings fell while deceased was cleaning away the rubbish produced by a shot fired the night before. The stone was undoubtedly loosened by the shot and should have been taken down before commencing to clear away the rubbish. Two feet nine seam.

SMITH John - 1904 - 21/11/1904 – Age 54 - Houghton Shaftsman, Lambton Collieries Ltd., Co.Durham. He was assisting at starting a haulage road when the tail rope was a little slack and sprung across the road when the set was started and struck him.

SMITH John - 1905 - 01/05/1905 – Age 29 - Exhall Dataller, Exhall Colliery Co., Warwickshire. A train of six empty tubs broke away down an incline of 1 in 13 and ran into another train of stationary tubs being filled by the deceased. He was fatally crushed and died the same day.

SMITH John - 1905 - 11/09/1905 – Age 40 - Woolton No.2, Sandstone Quarry Labourer, Oliver and Co., Lancaster, Lancashire. While he was moving a loaded waggon, by means of a pinchbar, the load turned on to him and inflicted fatal injuries.

SMITH John - 1906 - 10/11/1906 – Age 69 - Newbattle Pumper, Lothian Coal Co. Ltd., Edinburgh. Owing to repairs in the shaft deceased was unable to descend and went to go to work a pump, of which he had charge, by an in-going-eye from the surface. He did not get to the pump, and was missed for a day and search was made, when he was found at the entrance to the in-going-eye quite dead. His death was due to natural causes.
